Helping hospitals to help you

Sharing key information helps hospital staff understand how best to care for someone.
When someone goes into hospital, it helps staff if they have important information about their care and wishes.

This may include a ReSPECT form. This explains what care and treatment a person wants in an emergency, especially if they cannot speak for themselves. People may also have care plans covering:

  • medicines
  • eating and drinking
  • moving around
  • ongoing health conditions

People with a learning disability or who are autistic may have a Care Passport. This explains their needs and how they communicate. Care Passports can be used in any care setting, including hospitals.
